CLIENT INTAKE FORM
This intake form helps us get to know you and your dog before your first session. By completing it in detail, you'll give us important background on your dog's health, activity level, and goals so we can create a safe, personalized fitness and conditioning plan. Please fill out all sections to the best of your ability.
ADDITIONAL DOG FORM
This form is for clients who have already completed the above form and have additional dogs they would also like to participate in Fetch and Flex's programs.
